At Montgomery Kidney Specialists, we offers program support and workshops to help our patients further understand what Chronic Kidney Disease is and how to help maintain their health.

Remote Patient Monitoring (RPM)
High blood pressure, or Hypertension, is the most common chronic disease and a major risk factor for cardiovascular disease, stroke, kidney failure, and other diseases. That is why we offer the RPM Program for our patients. Remote patient monitoring allows us to monitor your blood pressure from the comfort of your home. This allows us to manage acute and chronic conditions and it cuts down on patients' travel costs to your provider's office.

Kidney Disease Education (KDE)
Our office offers workshops to qualifying patients to help educate them about chronic kidney disease. KDE workshops are for patients with CKD stages 4 and 5 and/or End Stage Renal Disease (ESRD) and are receiving maintenance dialysis. The goal of this model is to create better outcomes, improve quality, and reduce costs for all of our patients.

Chronic Care Management (CCM)
CCM includes a comprehensive care plan that lists your health problems and goals, other providers, medications, community services you have and need, and other information about your health. It also explains the care you need and how your providers will coordinate it. By participating in this program, your provider will prepare a care plan for you or your caregiver which will help you with medication management, gives you support when you go from one health care setting to another, and can help you with other chronic care needs.

REACH Kidney Care
REACH is a kidney health management program designed to benefit members along the continuum of kidney disease, including advanced chronic kidney disease (CKD) and end-stage renal disease (ESRD). REACH provides one-on-one, individualized care management, both in-person and virtually, to help members navigate their health in a way that best fits their life.

Monogram Health
Monogram provides in-home, multispecialty care for patients with chronic kidney disease and related metabolic and pulmonary conditions. As multispecialty providers, they render 24/7 care, ensuring our patients receive essential and timely support without leaving their homes.

Dialysis
Our doctors provide care and services to all of our patients on dialysis. We are affiliated with many dialysis clinics in Central Alabama and can help you find a dialysis center close to you. There are three types of dialysis: Hemodialysis, Peritoneal Dialysis, and Home Hemodialysis. Hemodialysis is the most common type of dialysis and the one most people are aware of. Peritoneal dialysis uses the inside lining of your abdomen (the peritoneum) as the filter, rather than a machine. Home hemodialysis can be scheduled to a patient’s specific health condition and lifestyle, but often times requires a care partner to assist with the dialysis process.

Kidney Transplants
For patients with ESRD, a kidney transplant may be an alternative choice for patients with severe damage to their kidney(s). A successful kidney transplant can give you increased strength, stamina and energy. Our staff can help guide you in the right direction by getting your prepared and putting you on the path to receiving a healthy new kidney.
